Registered Nurse-Assessment Coordinator
The Registered Nurse Assessment Coordinator (RNAC) support facilitating all referrals to CLC programs by collecting significant data and information to ensure admissions are meeting treating specialty requirements. This position will utilize the CLC admission criteria for coordination of admissions and/or transfers to all CLC programs. This may involve travel to local facilities to assess if the Veteran's needs meet the criteria for admission to CLC. The RN Assessment Coordinator is responsible and accountable for ensuring accuracy of the Minimum Data Set (MDS) process to include timeliness of assessment and MDS processing, monitoring QM, SAIL data reporting, care planning, staff training on Caribou MDS system and ADL documentation. The RNAC manages and monitors electronic transmission of the MDS at designated intervals. Monitors and audits documentation of IDT members and nursing staff along with assessments for completion and accuracy which affects reimbursement. The RNAC is a subject matter expert in relation to admission criteria. Develops and communicates an appropriate plan of action to correct identified quality deficiencies. Provides education to IDT members and nursing staff as needed for accuracy. Work Schedule:

In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires a combination of walking, standing, stooping, kneeling, bending, pulling, and pushing with potentially long periods of sitting. Transferring patients and objects may occasionally be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or during patient procedures and care. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. Use of fingers, both hands, and near and far vision is required.
