Perform Inpatient Acute and Post- Acute assessments of members using clinical skills and appropriate measurement tools. Provide outcomes targets to appropriate audience.
Collaborate effectively with the members health care team to establish an optimal, individualized plan of care for transition to most appropriate setting along continuum. The health care team includes physicians, referral coordinators, health plan UM Nurse, hospital discharge planners and social workers, etc.
Assist the member in meeting short and long-term goals with regards to their overall well-being.
Consult with Medical Directors and/or management to resolve any barriers in the patients movement along the continuum of care.
Participate in discussions with health plans and physician groups to assess member population and their progress of care in the Acute/ Post- Acute settings.
Coordinate and monitor comprehensive health care including Acute/Post- Acute and community-based services.
Visit/engage member and/or caregiver initially and weekly thereafter (includes telephonic engagement).
Assess and monitor patients appropriateness for Post- Acute setting (as indicated) according to OPT recommendations and Interqual criteria and refer to licensed physicians for next level of care determinations.
Manage assigned caseload in an efficient and effective manner utilizing time management skills.
Provide constructive information to minimize barriers and increase member and family satisfaction as well as that of providers.
Enter timely and accurate data into designated care management applications.
Educate member, caregiver and facility peers on OPT (Outcome Prediction Tool) when possible.
Daily review of census and identification of barriers to manage independent workload and ability to cover/ assist others.
Review monthly dashboards, readmission reports, quarterly and other reports as needed to assist with the identification of opportunities for improvement.
Adhere to organizational and departmental policies and procedures.Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
