Resident Resource Coordinator

Located in Erie, PA, Housing and Neighborhood Development Service (HANDS) is a mission based entrepreneurial non-profit agency dedicated to providing affordable and appropriate housing for persons who are in need of safe and affordable housing.
Under the direct supervision of the Director of Property Management, the Resident Resource Coordinator connects residents of HANDS properties with social support services and/or sources of funds to help them pay bills such as utilities and to help maintain their apartments.
This is a full-time position.
Duties and
Responsibilities:
Establish working relationships with agencies and service providers in the community.
Generates and periodically updates a directory of providers for use by both project staff and residents.
Refer and link the residents of the project to service providers in the general community; these are, for example, case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, occasional visiting nurse, preventive health screening/wellness and legal advocacy.
Educate residents on service availability, application procedures, client rights, etc.

Monitor the ongoing provision of services from community agencies, keeping the case management and provider agency current with the progress of the individual.
Manages the provision of supportive services where appropriate.
Set up volunteer support programs with service organizations in the community.
Help the residents build informal support networks with other residents, family and friend.
As needed, may counsel residents in the obligations of tenancy.
Educate other staff on the management team on issues related to aging in place and service coordination, to help them to better work with and assist the residents.
Qualifications:
Bachelors of Social Work from an accredited school of social work or related degree.

years' experience required, to include counseling, case management, and working in group homes.
Excellent interpersonal relationship skills.
Working knowledge of available resources and services that will help HANDS residents live well in the community.
Ability to administer financial literacy and budget counseling.
Familiar with symptoms associated with various disabilities and debilitating conditions including, but not limited to, the following conditions/processes/disabilities:
substance abuse, mental illness, medical/physical/neurological conditions, gerontology, poverty, unemployment, post-traumatic stress syndrome (PTSD), the grieving process, the adjustment process, loss, adjustment to change, HIV/Aids, and the development and emotional effects of stigmatization as a result of the conditions.
Valid driver's license, car insurance, reliable personal automobile.
